ABSTRACT

Objective To analyze the present post setting situation of CDCs in Zhejiang Province, and to provide evidence for further improvement and standardization of post setting. Methods The data of post setting were collected by questionnaires from all levels of CDCs in Zhejiang Province. The compliance and rationality of post setting structure were evaluated by national and provincial normative policy documents. Results Ninety seven CDCs had implemented the post setting. Professional technical posts had the highest proportion (above 80%) among three kinds of posts. As the approved proportions of posts classification and professional titles were approved by the local government, there were differences between different levels of CDCs. The approved proportion of ground skilled posts and management posts between municipal and county CDCs had a relatively higher degree of dispersion, and the CVs of ground skilled posts and management posts of municipal CDCs were 72.3% and 65.6%, and the CVs of county CDCs were 101.6% and 82.1%. In terms of the approved professional titles structural proportion, the degree of dispersion appeared to be higher in senior titles and primary titles approved proportion of municipal CDCs (CV=29.1%, 28.5%), while the degree of dispersion was higher in senior titles and vice-senior titles of county CDCs (CV=58.9%, 21.8%) . Meanwhile, the approved proportions of senior titles in municipal and county CDCs were lower with the averages of 32.2% and 17.0%. The posts of infectious disease prevention and control and laboratory detection were accounting for the majority posts in municipal and county CDCs with average percentages of 47.8% and 45.1% . Conclusion The present situation of post structural proportion meets the relevant requirements generally. However, there are some phenomena of non-standard approved proportion of ground skilled posts and management posts and low senior titles approved proportion. Meanwhile, the percentage of infectious disease prevention and control and laboratory detection posts appears to be lower in county CDCs.

ABSTRACT

Objective To evaluate the accuracy of renal scintigraphy for the estimation of glomerular filtration rates (dGFR) in patients with diabetic nephropathy as compared to the conventional dual-plasma sample clearance method (pscGFR). Methods Forty-six patients with diabetic nephropathy underwent both dynamic renal scintigraphy and dual-plasma sample measurement after 99Tcm-DTPA injection. Paired student t-test and correlation analysis were performed to compare dGFR and pscGFR (normalized to body surface area,1.73 m-2). Results The mean dGFR was higher than mean pscGFR ((51.08±26.78)ml·min-1vs (44.06±29.43)ml·min-1,t=4.209,P=0.000). The dGFR correlated with pscGFR ( r=0.923,P=0.000) linearly (regression equation:pscGFR=1.015×dGFR-7.773,F=254. 656,P=0.000).Conclusions dGFR correlated well with pscGFR. Although it could not absolutely replace the latter in patients with diabetic nephropathy,dGFR could reasonably evaluate the filtration function for these patients.

ABSTRACT

OBJECTIVETo study the pathological alteration of the cochlear microcirculation and the role of endothelin (ET) in the process of the noise-induced injury of inner ear.

METHODSThirty rats were randomly divided into five groups: control group and 1, 4, 8, 15 d noise exposure groups (115 dB, white noise, 8 h daily). The cilium of hair cell was observed by scanning electron microscope, the cochlear microcirculation was determined by stretched preparation of the stria vascularis. The level of ET in plasma was measured through radio-immunity. The distribution of ET-1, ETA, ETB, in cochlea were detected through immunohistochemistry staining.

RESULTSIn the 4, 8, 15 d group, severe ischemia appeared on the capillary of stria vascularis and the cilium of hair cell displayed significantly disorder. The level of ET in plasma rose temporarily in the 4 d group. ET-1 activity distributed widely in the rat cochlea, there was no significant difference between the control group and the noise exposure groups. ETA expressed in the plasma of the intermediate cells and the capillary walls in the stria vascularis. The control group and the 1 d group showed weak positive staining, while the 4, 8, 15 d groups showed strong positive. The ETB, activity distributed on the endothelial cells of the capillary of stria vascularis and the alteration of the staining intensity was similar to the manifestation of ETA.

CONCLUSIONSSevere disturbance of the cochlear circulation occurred during the course of the noise injury to the inner ear. At the same time, the activity of the ET system of the cochlea stepped up significantly. It coincided with the ischemia of the stria vascularis. These findings suggest that ET may play an important role in the process of the cochlear microcirculation disorder caused by noise.

ABSTRACT

AIMTo study the integration of hepatitis B virus (HBV) DNA into sperm chromosomes in hepatitis B patients and the features of its integration.

METHODSSperm chromosomes of 14 subjects (5 healthy controls and 9 HB patients, including 1 acute hepatitis B, 2 chronic active hepatitis B, 4 chronic persistent hepatitis B, 2 HBsAg chronic carriers with no clinical symptoms) were prepared using interspecific in vitro fertilization between zona-free hamster oocytes and human spermatozoa. Fluorescence in situ hybridization (FISH) to sperm chromosome spreads was carried out with biotin-labeled full length HBV DNA probe to detect the specific HBV DNA sequences in the sperm chromosomes.

RESULTSSpecific fluorescent signal spots for HBV DNA were seen in sperm chromosomes of one patient with chronic persistent hepatitis B. In 9 (9/42) sperm chromosome complements containing fluorescent signal spots, one presented 5 obvious FISH spots and the others 2 to 4 signals. The fluorescence intensity showed significant difference among the signal spots. The distribution of signal sites among chromosomes seems to be random.

CONCLUSIONHBV could integrate into human sperm chromosomes. Results suggest that the possibility of vertical transmission of HBV via the germ line to the next generation is present.
